To Qualify for The Ultimate Cash Incentive Program:

To Qualify as an existing customer for this Ultimate Program you must have placed an order for (25) or more cheer bows from THE ULTIMATE BOW.
Existing Customers must complete the above form in its entirety, prior to the New Customer placing their order.
New Customer must provide the Existing Customer's Name on the Checkout Page at the time the order is placed; they cannot provide the existing customer name after the order is placed to qualify.
One Time Offer per New Customer/Cheer Organization.
Existing Customers can refer as many new Customers/Cheer Organizations to THE ULTIMATE BOW as they like.
If the New Customer/Cheer Organization is already in our database as an existing customer, the cheer bow order will not qualify for the Ultimate Cash Incentive Program.
